Abstract Among the various medications, antipsychotics are most frequently reported to cause galactorrhea. However, galactorrhea with antidepressants, including selective serotonin reuptake inhibitors, is less commonly reported. We report a 37-year-old female who was treated for generalized anxiety disorder with paroxetine and developed hyperprolactinemic galactorrhea. As a result, paroxetine was cross-titrated with escitalopram. The patient showed significant improvement in anxiety symptoms, and her galactorrhea completely resolved upon switching the medication from paroxetine to escitalopram. This case highlights the importance of clinician awareness regarding the potential for galactorrhea to occur with paroxetine, enabling comprehensive and timely management.
