Abstract: Diabetes mellitus is a chronic metabolic disorder that is marked by persistent hyperglycemia due to an abnormality in insulin secretion, insulin sensitivity, or both. As one of the most rapidly growing health concerns worldwide, diabetes is known to be linked with serious vascular complications such as nephropathy, neuropathy, and retinopathy. The conventional treatment strategies for diabetes are mainly based on insulin therapy, oral hypoglycemic agents, and dietary regulation. However, the toxicities associated with allopathic medications and the requirement for supportive vascular therapies have led to an increasing interest in alternative or natural therapies. Horse chestnut, an ancient medicinal plant that has been used in traditional medicine for centuries, has been identified as a potential natural remedy for the vascular complications of diabetes. The seeds and bark of this plant are known to be rich in bioactive compounds, especially escin, flavonoids, and coumarins, which possess antioxidant, anti-inflammatory, vasoprotective, and antithrombotic properties. These properties make it an effective agent for the treatment of chronic venous insufficiency, edema, and diabetic nephropathy, which are common complications of diabetes. This review aims to examine the biochemical, pathological, and genetic mechanisms of diabetes mellitus and evaluate the pharmacological properties of horse chestnut that justify its use as an alternative therapeutic agent. This review also aims to examine the recent preclinical and clinical studies on the use of horse chestnut extracts in the management of diabetes and emphasize the need for further research on the mechanisms of its efficacy. By integrating modern endocrinological knowledge with ethnobotanical knowledge, this review proposes a holistic approach to the management of diabetes that combines conventional and herbal therapies.
