Minimally invasive transperitoneal laparoscopic adrenalectomy is a safe and effective approach for large adrenal cystic lesions.
Background: Adrenal lymphangiomas are rare, benign, cystic tumors of lymphatic origin. They are often incidentally discovered and pose diagnostic challenges due to nonspecific imaging features. Definitive diagnosis requires surgical excision and histopathology. Case presentation: A 52-year-old woman with hypothyroidism was found to have a progressively enlarging right adrenal mass. Imaging revealed a large multiloculated cystic lesion with septal enhancement, hemorrhage, and calcifications, consistent with adrenal lymphangioma. Functional adrenal disease was excluded. She underwent transperitoneal laparoscopic adrenalectomy with complete excision. Histopathology confirmed adrenal lymphangioma. Recovery was uneventful, and she remained asymptomatic at the 3-month follow-up. Conclusion: Minimally invasive transperitoneal laparoscopic adrenalectomy is a safe and effective approach for large adrenal cystic lesions in experienced centers.
