Population
A 64-year-old woman with rheumatic heart disease, previous mechanical aortic and mitral valve replacements, redo mitral valve replacement, and tricuspid valve annuloplasty, presenting with tachy-brady syndrome and sinus node dysfunction.

Intervention
Atrial leadless pacemaker (Aveir AR) implantation, programmed to AAIR 60-130, added to an existing ventricular leadless pacemaker (Micra AV) programmed to VVI 40.

Resolution of pacemaker syndrome symptoms and maintenance of stable pacing parameterspatient reported
Atrial leadless pacing is a feasible and effective treatment for pacemaker syndrome induced by ventricular leadless pacing in patients with sinus node dysfunction.
