SUMMARY: Diagnostic errors in pediatric neuroradiology remain underrecognized despite their potential impact on patient care. This review expands upon existing frameworks by integrating cognitive, procedural, and communication-related contributors to diagnostic error, with the goal of identifying actionable strategies for improvement. Illustrative cases were selected from a pediatric neuroradiology peer-learning database at a high-volume academic center. Errors were categorized into four domains: perceptual, interpretive, process-related, and communication-related. For each case, contributing cognitive biases and system-level failure points were analyzed, and potential mitigation strategies were identified based on peer discussion and relevant literature.
