A 48-year-old female with a phthisical right eye presented with persistent discharge and cicatricial entropion involving both upper and lower eyelids. She was diagnosed as a case of right nasolacrimal duct obstruction and in view of the eye being phthisical she underwent dacryocystectomy during which the lacrimal sac mucosa of the excised lacrimal sac was used as a mucosal graft for correction of her cicatricial entropion. This innovative use of autologous tissue provided effective correction of the entropion with a satisfactory cosmetic outcome. The structural similarity between lacrimal sac and conjunctival mucosa supports its use as a replacement of posterior lamella. To the best of authors' knowledge, this is the first reported case of utilizing lacrimal sac mucosa as a graft material for replacing the posterior lamella.
