Abstract Introduction Obesity is considered a separate endotype in children with asthma. The aim of this research was to evaluate the lung volumes in asthmatic children with and without obesity and relationship of these volumes with asthma control. Methods We enrolled asthmatic children who were overweight/obese to the case group and those who were normal weight to the control group. Clinical and sociodemographic characteristics as well as asthma control test (ACT) was recorded at enrollment and the patients underwent plethysmography. FEV1, FVC, FEV1/FVC, FRC, ERV, RV, TLC, IC, VC z-scores calculated using GLI lung function calculator were compared between overweight/obese versus normal weight groups. Results We enrolled 193 asthmatic subjects; 77 (52 male) overweight/obese and 116 (71 male) with normal weight. Median ages in both groups were 12 years but age at diagnosis was significantly higher in the overweight/obese group with a median of 63 months (p = 0.02). Asthma was well controlled in 23.5% of the overweight/obese and 24.5% of the normal weight groups (p = 0.85). Spirometry results revealed that FEV1/FVC z-scores were significantly lower in the overweight/obese group compared to the normal weights (p = 0.005). Overweight/obese subjects had significantly lower ERV and FRC compared to normal weight group (0.001). IC and VC z-scores were significantly higher in the overweight/obese compared to the normal weight group (-0.236 and -2.47 versus -0.44 and -3.06 respectively, p 0.001 for both). Conclusion We have demonstrated lower FRC and ERV but higher IC and VC in asthmatic children with obesity compared to asthmatic children with normal weight despite similar asthma control.
