A 30-year-old male presented with retrosternal chest pain after receiving a check during a recreational hockey game. An ECG was suggestive of a STEMI and he suffered from a VF arrest. Catheterization demonstrated occlusion of his LAD, secondary to LM coronary dissection. He developed progressive shock and went for emergent CABG. There are 10 cases of traumatic LM dissection presented in the literature, although none occurred during sport. This highlights the importance of maintaining a wide differential in young patients with potential traumatic injury to the chest wall when presenting to the ED with chest pain.
