ABSTRACT Clostridium perfringens and Clostridium ramosum are anaerobic, Gram-positive bacilli commonly found in the gastrointestinal tracts, which can lead to life-threatening infections and sepsis under certain conditions. We present a case of a 60-year-old man with a history of chronic constipation who presented with hypotension, altered mental status, and respiratory distress and was found to have bacteremia due to Clostridium perfringens and Clostridium ramosum in the setting of stercoral colitis, a rare and serious condition caused by fecal impaction leading to colonic ischemia.
