Osteogenesis imperfecta (OI) is a group of genetic disorders characterized by fragile bones that easily break. Anesthetic management of patients with OI is challenging due to skeletal fragility, risk of fractures, and potential airway difficulties. Strategies that minimize airway manipulation and reduce opioid exposure are particularly relevant. A 10-year-old child with OI underwent femoral telescopic nail revision. General anesthesia with supraglottic airway combined with femoral nerve and sacral erector spinae plane blocks provided effective intra- and postoperative pain management without opioids. This case highlights the feasibility of combining airway-sparing and opioid-sparing strategies in pediatric OI.
