Background: Sinonasal ossifying fibroma (OF) is a rare benign fibro-osseous tumor that can cause locally aggressive disease of the paranasal sinuses and skull base. Data on the outcomes after contemporary endoscopic management remain limited. Methods: We retrospectively reviewed patients with pathologically confirmed sinonasal OF diagnosed at a tertiary referral center. Seven patients were included in this study. Four patients who underwent definitive endoscopic endonasal surgery with complete resection and postoperative follow-up were included in the outcome analyses. Results: The lesions arose from the ethmoid sinus (n=2), maxillary sinus (n=1), and sphenoid sinus (n=1). All tumors had well-defined margins and a mixed-density pattern on computed tomography. All patients underwent endoscopic endonasal resection with complete excision. After a median follow-up of 24 months (range: 3–69 mo), no recurrence was observed, and none showed evidence of disease during the last follow-up. Conclusion: Complete endoscopic resection of the sinonasal OF achieved excellent short- to mid-term disease control without major morbidity.
