Abstract

Focused ultrasound is a noninvasive, image-guided therapeutic technology with emerging promise for the treatment of facet joint arthritis, a major contributor to chronic low back pain. Conventional interventions such as steroid injections, radiofrequency (RF) neurotomy, and cooled RF lesioning are invasive, rely on fluoroscopic guidance, and are limited by diagnostic uncertainty, procedural risk, and variable long-term efficacy. This paper presents a comprehensive review of current treatment modalities and introduces a novel, feedback-controlled focused ultrasound system engineered for precise nerve ablation or neuromodulation. The system integrates maximum power point tracking (MPPT) to maintain ultrasonic resonance and employs acoustic feedback via cavitation-induced subharmonics to guide real-time energy delivery. Bench validation demonstrates robust frequency tracking, sustained peak acoustic output, and reliable detection of cavitation onset. Comparative analysis from literature review for RF neurotomy reveals focused ultrasound offers similar or superior efficacy (pain relief ≥50% in 80–85% of cases at 6 months), with enhanced safety, patient compliance, and repeatability. The integration of our responsive control and noninvasive delivery positions focused ultrasound as a transformative alternative for managing facet-mediated spinal pain, potentially reducing the cost of care and improving quality of life.
