Abstract

サルコイドーシスは,全身臓器に非乾酪性類上皮細胞肉芽腫を形成する疾患で,本邦では呼吸器病変,眼病変,心臓病変の頻度が高く,死亡例の多くは心臓病変に起因する.病変の糖代謝を反映するFDG PET/CT検査は,心臓病変の活動性評価において保険適用となっているが,一度に全身撮像を行うため,心外病変の評価にも適している.通常FDG投与約60分後より撮像するstatic画像を用いて,治療前評価,免疫抑制療法の効果判定,再発診断を行うが,半導体検出器を備えたPET/CT撮像機器の登場によりdynamic PET撮像が可能となり,がんや炎症性疾患への臨床応用が期待される.本稿ではサルコイドーシスにおける治療前のFDG-PET/CT所見,FDG高集積を呈する他疾患との鑑別,治療後の集積変化,予後に関して心臓病変,心臓外病変の相違を含めて概説し,当施設のdynamic FDG-PET/CTにおけるサルコイドーシスを含むいくつかの臨床的知見を紹介する.
