Abstract

We apply the Identity Axis Framework to schizophrenia, showing it is categorically distinct from all prior framework entries. It is not cellular identity drift producing a local failure; it is a three-level cascade in which parvalbumin (PV) interneuron identity loss (EZH2/H3K27me3 at PVALB/SST/GAD67 loci silencing LHX6/NKX2-1) produces cortical gamma coherence collapse, which produces a cognitive false attractor: the confabulated reality model (hallucinations and delusions). The hallucination is not the disease. It is the solution: the most coherent narrative a coherence-seeking system can generate from incoherent circuit noise. When the Hub lock is released and the Identity Anchor restored, the noise disappears and the confabulation collapses radically and irrevocably --- not gradually. We derive the full N-dimensional presentation manifold: 3 dimensions for the emergence condition (genetic depth, developmental pruning trajectory, external perturbation); additional dimensions for the presentation signature (which of five circuit identity systems have co-failed). We derive the first geometric account of catatonia as independent Motor Identity System (Circuit 2) failure, explaining its occurrence across diagnostic categories. We identify the 22q11.2 deletion as a constitutive multi-circuit failure proof. We derive a dimension-specific integrated treatment protocol: TDI-6118 (CNS-penetrant EZH2 inhibitor) targeting the cellular Hub lock; LHX6-directed AAV for Identity Anchor restoration; 40 Hz sensory entrainment (GENUS) for circuit rehabilitation; antipsychotics repositioned as bridging agents only. Nine falsifiable predictions are stated and timestamped 2026-03-09.
