Associations of Sleep Duration and Quality with Suicidal Ideation among Korean Older Adults

Abstract

본 연구는 국내 노인 자살 예방을 위해 수면 시간 및 질과 자살 생각 간의 관련성을 분석하여 정책 수립의 기초 자료를 제공하고자 수행되었다. 2024년 지역사회건강조사 자료를 활용하여 65세 이상 노인 81,323명을 대상으로 복합표본 로지스틱 회귀분석을 실시하였다. 수면시간(단기, 정상, 장기)과 PSQI를 활용한 수면의 질을 독립변수로 설정하여 연관성을 분석하였다. 분석 결과, 대상자의 10.1%가 자살 생각을 경험하였다. 정상수면 대비 단기수면 군(aOR=1.18, 95% CI: 1.09-1.28), 수면 질이 좋은 군 대비 나쁜 군(aOR=1.81, 95% CI: 1.55-2.11)에서 자살 생각 위험이 유의하게 높았다. 특히 수면의 양적 부족보다 질적 저하가 자살 생각과 더욱 긴밀한 연관성을 보였으며, 이러한 양상은 우울 증상군, 정신건강 상담 미경험군, 기혼 노인 집단에서 더욱 현저하였다. 본 결과는 고위험군 선별 시 수면의 질을 핵심 평가 지표로 고려해야 함을 시사하며, 정신건강 취약계층 및 기혼 노인 대상의 다각적 중재 연구가 필요할 것이다.
