The objective of this present study was to evaluate the perioperative and functional outcomes associated with SP Retroperitoneal Robotic Partial Nephrectomy (RAPN) for renal cell carcinoma (RCC). METHODS: A retrospective review was performed on the prospectively maintained, IRB-approved multi-institutional database of the SP Advanced Research Consortium (SPARC) to identify 94 patients who underwent SP-RAPN for RCC between 2019 and 2023. Baseline clinicodemographic, perioperative, and postoperative outcomes were analyzed using R Project for Statistical Computing with descriptive statistics as presented. RESULTS: All procedures were successfully completed using the SP robotic platform without any need for conversion or intraoperative complication. The median operating time and estimated blood loss were 161 minutes and 50 mL, respectively. Warm ischemia was pursued in 99 patient (86.8%) with a median ischemia time of 15.7 minutes. Surgical drain tube was not required in 95.8%. Major postoperative complication of Clavien-Dindo grades ³3a was identified in two patients, both pertaining to pseudoaneurysm. At a median follow-up duration of 12 months, we did not identify any disease recurrence and clinically significant CKD upstaging was only identified in three patients. CONCLUSIONS: Retroperitoneal SP-RAPN was associated with favorable perioperative outcomes with low risk of perioperative morbidity and RCC recurrence. Continuous advancements in operating techniques using the SP robotic platform, such as with the introduction of the floating-docking technique and low anterior access for retroperitoneal SP-RAPN, will likely result in further improvements in patient outcomes.
