In this study, we investigated perioperative results of robot-assisted partial nephrectomy (RAPN) in patients with CKD. METHODS: A multicentric dataset of 2059 RAPN was retrospectively analyzed. CKD stage was defined according to KDIGO 2012 criteria and patients were dichotomized in moderate–severe CKD (Stage III-V; eGFR60 ml/min). RESULTS: 17% of patients revealed a baseline eGFR0.05). Concerning tumor characteristics, there was no difference in tumor size, but patients with mild or normal CKD had higher PADUA scores (9 vs. 8; p 60 ml/min (43% vs. 34%; p=0.002). However, major complications (Clavien Dindo>2 ) occurred more often in patients with limited kidney function (10% vs. 7%; p0.05). Patients with eGFR<59 ml/min remained higher proportion of acute kidney failure according to RIFLE criteria (risk: 26% vs. 20%; injury: 4% vs. 2%, failure: 3% vs. 0.3%; p<0.001). MIC criteria (negative margins, ischemia time<20 minutes, no major complications) were achieved in the majority of patients without differences between both groups (75% vs. 79%; p=0.112). CONCLUSIONS: RAPN is a good option for patients with lower baseline eGFR. The renal function loss stays within tolerable ranges. To set realistic patient expectations, this patient group should be informed preoperatively about a higher risk for perioperative complications.
