Abstract

Summary The advent of comprehensive genomic profiling has revolutionized not only the identification of the tissue of origin in cancer of unknown primary (CUP) but also enabled the detection of potentially actionable molecular alterations. Recent evidence demonstrates that molecularly targeted therapies improve outcomes in patients with unfavorable CUP harboring such alterations. This review summarizes current methods for determining the tissue of origin and discusses recent clinical trials evaluating molecularly guided therapies in unfavorable CUP.
