Key Points

  • The study aims to evaluate the effects of Korean medicine on Crohn’s disease activity and clinical outcomes in a specific patient.
  • Reported clinical outcome in a 49-year-old male with recurrent bleeding.
  • Treated with Gami-Guibi-tang and acupuncture.
  • Monitored with Crohn’s Disease Activity Index (CDAI).
  • CDAI score decreased from 230 to 60 over six months.
  • Hematochezia resolved after initiation of the treatment.
  • Significant reduction in abdominal pain observed without adverse events.

Abstract

Objective: We report changes in the Crohn’s Disease Activity Index (CDAI) and clinical outcomes after Korean medicine treatment, including Gami-Guibi-tang and acupuncture, in a patient with active Crohn’s disease. Methods: A 49-year-old male with recurrent bleeding despite upadacitinib 15 mg once daily presented to our clinic in March 2025.Results: The CDAI decreased from 230 at baseline (2025-03-07) to 60 by 2025-09-15. After initiation of Gami-Guibi-tang on 2025-07-10 with weekly acupuncture, hematochezia resolved and abdominal pain (VAS) improved. No adverse events were observed. Conclusion: Korean medicine treatment centered on Gami-Guibi-tang may contribute to improvement in disease activity in patients with active Crohn’s disease with bleeding.
