Abstract Stroke in young people can have serious physical, emotional and social effects. This case involves a 41-year-old man with no previous health issues who experienced a large right posterior circulation stroke affecting the posterior cerebral artery territory. He had severe neurological deficits and complications such as aspiration pneumonia, which required ventilator support and a tracheostomy, leading to extended rehabilitation to recover and prevent further problems. A family history of young stroke prompted additional tests, but financial constraints prevented a definitive diagnosis. This case highlights the importance of combining neurorehabilitation and neuropalliative care in managing young stroke patients with unclear causes, focusing on early goal setting, functional recovery, clear communication and advanced care planning.
