Abstract

要旨 【目的】 Acute respiratory distress syndrome(ARDS)の深鎮静管理の筋弛緩薬使用が急性期の筋肉量へ与える影響は不明である。本研究では,COVID–19重症例でアミノステロイド系筋弛緩薬(以下,AS系NMB)使用が筋肉量に与える影響を単施設後方視的にCTで評価した。 【方法】 2020年4月から2023年7月までに当院ICUに入室し,深鎮静管理を行ったCOVID–19によるARDS例のうち,入室時と入院中の第3腰椎(L3)レベルのCTを撮像した患者を対象とした。鎮痛薬・鎮静薬で自発呼吸が抑制できない場合,AS系NMBを使用した。L3レベルの大腰筋・傍脊柱筋の水平・垂直距離の積和(CSA)を筋肉量の指標として,AS系NMB使用群と対照群でCT間の筋断面積減少率を比較した。 【結果】 Albumin,蛋白投与量を除き,患者背景,主な治療介入に有意差はなかった。主要評価項目である大腰筋・傍脊柱筋のCSAの1日あたりの筋断面積減少率は,AS系NMB使用群で中央値0.73%,対照群で0.43%であり,両群間に有意差を認めなかった(p=0.94)。 【結語】 COVID–19重症例の深鎮静管理において,AS系NMBの使用は急性期の筋断面積減少に有意な影響を及ぼさない可能性が示唆された。ARDSにおける筋弛緩薬の至適使用法の確立にさらなる検証が望まれる。
