Abstract

This case report describes a 67-year-old male who presented with acute painful right eye and persistent vision loss 2 days after receiving intravitreal faricimab injection with a recurrence of symptoms noted at 2 weeks. His visual acuity decreased to 20/125-1, and the intraocular pressure (IOP) elevated to 38 mmHg. Ophthalmic findings revealed anterior chamber cells and vitreous haze which were consistent with hypertensive anterior and intermediate uveitis. Fluorescein angiography and indocyanine green angiography confirmed the presence of posterior vascular leakage. Symptoms were improved with oral and topical corticosteroids and IOP-lowering medications. The inflammation was successfully managed, leading to recovery of vision after completing treatment. This case highlights that hypertensive uveitis is a rare event that could occur following intravitreal faricimab injection, underscoring the need for close IOP monitoring and complete ophthalmic examination throughout the course of treatment to identify and manage potential vision-threatening complications.
