Abstract: PURPOSE: The purpose of the study was to evaluate the effect of latanoprostene bunod (LBN) 0.024% on the microcirculation of the optic nerve head (ONH) using laser speckle flowgraphy (LSFG). MATERIALS AND METHODS: We enrolled 35 eyes of 20 patients diagnosed with open-angle glaucoma (OAG). All patients received a baseline LSFG evaluation of ONH microcirculation, then one drop of topical LBN was instilled in the examined eyes at the clinic. LSFG measurements were repeated at 30 min and 2 h post-LBN instillation. Patients would continuously use LBN once every night for 1 month and return to the clinic again for another LSFG exam. RESULTS: The averaged mean blur rate of vessel area (MBRv) of ONH, which was 100.0% at baseline, significantly increased at 2 h (114.36 ± 20.02%, P < 0.05) and 1 month (113.91 ± 26.53%, P < 0.05) post-LBN instillation. The mean intraocular pressure (IOP) significantly reduced from 17.68 ± 4.11 mmHg at baseline to 12.02 ± 5.31 mmHg after 1 month of LBN use. CONCLUSION: Topical LBN 0.024% promotes ONH blood velocity. The LSFG is a timely and effective device in following the IOP-lowering effect in patients with OAG treated with LBN.
