Key Points

  • The aim is to explore various neurological manifestations of tuberculosis and their complexities in diagnosis and treatment.
  • Presented three individual case reports of CNS tuberculosis
  • Included details on imaging and bacterial diagnosis
  • Assessed pulmonary involvements and serious illness
  • Identified unique presentations of CNS tuberculosis in three cases
  • Highlighted difficulties in confirming bacterial specimens in low-resource settings
  • Demonstrated the importance of imaging in diagnosis and prognosis

Abstract

Central nervous system (CNS) – Tuberculosis (TB) is a major public health problem in developing countries such as India. It accounts for 5%–10% of cases of extra pulmonary TB, of which 1% have serious illness. The diagnosis and treatment of these patients is quite challenging in resource-limited settings because confirming a bacterial specimen is difficult. Imaging plays a crucial role in differentiating tuberculoma from other similar space-occupying lesions and in assessing the prognosis. Bacteriological demonstration of TB bacilli confirms the diagnosis. We present three unique presentations of CNS TB with two pulmonary involvements and serious illness.
