ABSTRACT Guide catheter extensions (GCE) represent a useful tool in percutaneous coronary interventions (PCI), allowing to increase support and successfully deliver material through calcific lesions or tortuous anatomy. We present the first case of fracture of the metallic collar braid of a Guidezilla TM II (Boston Scientific, Natick, MA, USA) GCE, remaining trapped in a severely calcified left anterior descending artery of a patient presenting with ST‐segment elevation myocardial infarction. The fragment was retrieved from the coronary artery with snaring technique, and the PCI was competed with conventional stenting without further complications.
