Extensor carpi ulnaris subsheath injuries range from instability, painful snapping, to chronic tenosynovitis. While acute cases often respond to immobilization, surgical intervention is required for refractory cases. This study introduces a novel reconstruction technique using a fascia lata allograft to create an anatomical, radial-based sling under the extensor reticulum. This approach avoids autograft harvesting, preserves the original tendon gliding pathway, is applicable to all types of injury, and enables simultaneous treatment of intrinsic extensor carpi ulnaris pathologies and ulnocarpal injuries. This technique offers a promising alternative to existing procedures, minimizing complications such as tenosynovitis and optimizing clinical outcomes.
