Mutrashmari is a disorder of the Mutravaha Srotas characterized by the formation of urinary calculi anywhere in the urinary tract, from the kidneys to the bladder. In modern medicine, it is referred to as urolithiasis, while smaller granular particles are described in Ayurveda as Sharkara or Sikata. The disease shows a clear male predominance (male:female ≈ 2:1) and poses a significant medico-surgical and economic burden due to its recurrent nature. Acharya Sushruta has included Ashmari among the Ashtamahagada highlighting its severity and potential for serious complications if not managed appropriately. Clinically, Mutrashmari presents with renal colic, pain in the loin and lower abdomen, dysuria, burning micturition, and altered urine characteristics. Although contemporary management options are effective but fail’s to prevent recurrence. Ayurvedic classics emphasize conservative management before surgical intervention, advocating the use of oral formulations with Chedana, Bhedana, Lekhana and Mutrala properties to facilitate disintegration and expulsion of calculi, along with strict adherence to pathya-apathya. In this case report, a 50-year-old male presented with low backache, bilateral loin pain with stiffness, abdominal bloating, burning micturition and urinary incontinence. The patient initially consulted an Allopathic surgeon, who advised Ultrasonography of the Whole Abdomen with special focus on the KUB region, which revealed bilateral Renal Calculi. Subsequently, the patient presented to the Kayachikitsa OPD at Government Ayurvedic Hospital, Erragadda, Hyderabad, on 14/12/2024. He was treated with shamana chikitsa using Hajarul Yahood Bhasma, Syrup Renalka, and Capsule Stonex for 40 days. Post-treatment evaluation showed complete relief from symptoms, with ultrasonography revealing complete clearance of calculi in the right kidney and reduction of the Left Renal Calculus from 10 mm to 3 mm.
