Trichosporon asahii infection in a lower limb stump: A diagnostic challenge in a severely burned patient.

Trichosporon asahii is an opportunistic yeast that colonizes the skin and mucous membranes but may cause severe invasive infections in critically ill patients. We report the case of an 81-year old man with extensive second- and third degree thermal burns (30-39% total body surface area) who, during the third week of intensive care, developed a deep infection of a previously amputated femoral stump. Cultures repeatedly yielded multidrug-resistant Pseudomonas aeruginosa and T. asahii, identified by MALDI-TOF mass spectrometry. Combined surgical management and antimicrobial therapy with meropenem and posaconazole led to clinical resolution and a favourable outcome.
