Key Points

  • To evaluate CT histogram analysis as a biomarker for differentiating malignant from benign anterior mediastinal masses.
  • used CT histogram analysis focusing on mean attenuation
  • assessed sensitivity and specificity for malignancy diagnosis
  • recommended adjunct use with clinical and morphologic assessments
  • identified need for prospective validation with advanced parameters
  • high sensitivity observed for histogram analysis
  • limited specificity for definitive diagnosis of malignancy
  • advocated for integration with clinical assessments for better decision-making

Abstract

CT histogram analysis, particularly mean attenuation, may serve as a complementary biomarker for differentiating benign from malignant anterior mediastinal masses. Given its high sensitivity but limited specificity, it should be used as an adjunct to clinical and morphologic assessment rather than as a determinant of surgical management. Prospective validation incorporating volumetric and advanced radiomic parameters is required.
