Uterine perforation, a rare complication during uterine surgery, poses diagnostic challenges due to its nonspecific nature. In this report, we present an exceptionally rare case of a postmenopausal female with diagnosed old uterine perforation adhesion involving the bowel on hysteroscopy and laparoscopy. The patient presented with complaints of vaginal bleeding and intermittent discharge. This case emphasizes the progression and outcome of symptoms associated with uterine perforation.
