In this national cohort of traumatic spinal injuries, chemoprophylaxis initiated ≥48 hours after admission was independently associated with increased venous thromboembolism and mortality. These findings support early initiation when clinically feasible while highlighting the need for prospective evaluation of bleeding risk.
