Successful surgical treatment of idiopathic chylothorax using near-infrared fluorescence imaging of indocyanine green

Abstract

成人の特発性乳糜胸は稀な疾患であり,治療に難渋することも少なくない.今回,icg蛍光法を活用することで,手術により治癒した症例を経験した.74歳,女性.咳嗽等を主訴に受診し,乳糜胸の診断で当科紹介となった.術前のリンパ管シンチグラフィー検査では胸管の異常分枝からの漏出が疑われた.保存的治療として胸腔ドレナージや脂肪制限を行ったが,乳糜は停止せず,手術を施行した.両側鼠径リンパ節へicgを注入した後,胸腔鏡手術を開始した.icg蛍光法で観察し,異常分枝とicgの漏出を確認できた.術前検査と照らし合わせ,異常分枝の盲端を漏出部と判断し,胸管本幹付近でクリッピングを行った.術後経過は良好で術後9日目に自宅退院となった.icg蛍光法は従来の脂肪負荷法と比較して視認性が高く,術前検査との併用により漏出部の確実な同定が可能となることから,特発性乳糜胸の外科治療において有用な手法であると考えられた.
