INTRODUCTION: Three-dimensional modeling technology has transformed gynecologic surgical planning, primarily benefiting surgeon understanding and operative confidence. However, limited data exists regarding the use of 3D modeling for direct patient education and empowerment. We present a novel approach utilizing patient-specific 3D fibroid models as counseling tools to enhance patient understanding, engagement, and shared decision-making. This innovative application addresses the gap between complex medical imaging and patient comprehension, potentially revolutionizing perioperative counseling in gynecologic surgery through personalized, interactive anatomical visualization. OBJECTIVE: To evaluate the impact of patient-specific 3D fibroid modeling on patient understanding, confidence, and empowerment during surgical counseling. Primary objectives included assessing changes in patient comprehension of their condition and surgical options before and after 3D model interaction. Secondary objectives examined effects on patient confidence in surgical decision-making, engagement in treatment discussions, surgeon-patient communication quality, and overall satisfaction with the counseling process. We aimed to demonstrate the feasibility and effectiveness of integrating 3D modeling technology into routine gynecologic patient counseling sessions. METHODS: We conducted a pilot case study involving a 36-year-old G1P1001 woman with symptomatic uterine fibroids. Patient-specific 3D models were constructed from standard MRI imaging data using advanced segmentation and reconstruction techniques. The interactive model featured transparent views, sagittal splitting capabilities, and individual fibroid highlighting with precise digital measurements. A structured counseling session was conducted using the 3D model to explain anatomy, FIGO staging, and treatment options. Patient understanding, confidence, and engagement were assessed using a comprehensive 16-item survey instrument administered before and after the 3D modeling session. Survey domains included comprehension, confidence, communication effectiveness, anxiety levels, and overall satisfaction with the counseling experience. RESULTS: The patient demonstrated dramatic improvement in understanding, rating her comprehension as “minimal” (1/5) before the 3D model and “excellent” (5/5) afterward. The 3D model significantly improved understanding of the surgical procedure and greatly increased confidence in undergoing surgery. Patient found the model extremely helpful (5/5) for visualizing fibroid location and size, particularly noting the “split in half” feature as most beneficial. Communication with the surgeon was rated as extremely effective (5/5), and the patient felt very comfortable discussing treatment options. Decision-making involvement was rated as extremely high (5/5), with greatly increased confidence in the surgeon's understanding of her case. While anxiety somewhat increased due to enhanced awareness of case complexity, the patient rated the overall experience as “very positive” and would strongly recommend 3D modeling to others. Most helpful features included color coding, transparency, and the ability to highlight individual fibroids. CONCLUSIONS: Patient-specific 3D fibroid modeling represents a transformative advancement in gynecologic patient counseling, dramatically improving patient understanding and engagement. This technology successfully bridges the gap between complex medical imaging and patient comprehension, fostering meaningful shared decision-making. The interactive nature of 3D models facilitates superior surgeon-patient communication and builds confidence in treatment decisions. Future applications should focus on expanding access, standardizing implementation protocols, and studying long-term patient outcomes to establish 3D modeling as a practice in gynecologic surgical counseling.
