Abstract

アトピー性皮膚炎(atopic dermatitis,AD)は乳児期に発症することが多く,先進国の小児の15~20%が罹患する.発症の約60%は生後1年以内にみられ,食物アレルギーや喘息などへ進展する「アレルギーマーチ」の起点となるため,新生児期における予防が重要となる.ADの発症には,皮膚バリア機能不全や免疫応答異常などの宿主因子に加え,衛生仮説に代表される環境要因や皮膚マイクロバイオームの多様性低下が複合的に関与する.特に,AD皮膚では黄色ブドウ球菌の異常増加を伴うdysbiosisが認められるが,それが原因か結果かは近年まで不明であった.乳児ADは1歳でアレルギー感作を伴うと3歳までの喘息発症リスクを大きく増加させることが報告されており,その予防はアレルギー疾患全体の発症抑制において重要である.本稿では,第62回日本小児アレルギー学会学術大会における発表内容を基に,スキンケアを中心とした乳児AD予防の最新知見と,皮膚マイクロバイオームを標的とした当グループの研究成果について概説する.
