Abstract: A 60-year-old woman with previous lung adenocarcinoma had bicytopenia and fragmented red blood cells (schistocytes) on a peripheral blood smear, indicating microangiopathic hemolytic anemia (MAHA). A bone marrow biopsy revealed metastatic adenocarcinoma. The identification of cancer-associated MAHA highlighted the significance of peripheral smear evaluation in distinguishing it from thrombotic thrombocytopenic purpura and targeting therapies toward underlying malignancy management.
