Key Points

  • This research aims to evaluate the effectiveness of advanced imaging and monitoring techniques during surgery for midbrain gliomas in children.
  • Described two clinical cases involving pediatric patients with midbrain tumors.
  • Utilized preoperative magnetic resonance tractography for surgical planning.
  • Employed intraoperative neurophysiological monitoring during the surgical procedure.
  • Assessed the approach to tumor resection and functional preservation.
  • Found that reconstruction of the corticospinal tract was enhanced by MR tractography.
  • Intraoperative monitoring effectively preserved the patients' functional status during surgery.
  • Indicated an optimal surgical approach was achievable with these techniques.

Abstract

This article describes two clinical cases of children with midbrain tumors who underwent surgery at the N.N. Burdenko National Medical Research Center of Neurosurgery. The efficacy of preoperative magnetic resonance (MR) tractography and intraoperative neurophysiological monitoring is examined. These modern techniques aid in determining the optimal surgical approach, extent of resection, and preservation of the patient’s functional status. Conclusion. Reconstruction of the corticospinal tract using MR tractography with subsequent intraoperative monitoring are effective methods in the surgical treatment of midbrain tumours.
