Cholangiocarcinoma (CCA) is the second most common primary hepatobiliary malignancy after hepatocellular carcinoma. Imaging plays a vital role in CCA diagnosis and staging. Initial evaluation typically includes imaging with CT or ultrasound, with MRI/MRCP providing additional utility in confirming diagnosis, evaluating disease burden, and excluding mimics of disease. This Pictorial Essay provides an overview of the CT and MRI manifestations of typical CCA presentations. Additionally, the key imaging features of entities that can mimic CCA on CT and MRI, including inflammatory conditions and neoplasms, are reviewed.
