Key Points

  • To evaluate the prevalence and characteristics of bacteremia in well-appearing children with prolonged fever attending the pediatric emergency department.
  • Observational study design
  • Sample included well-appearing children with prolonged fever
  • Assessment of bacterial diagnosis mainly through imaging
  • Clinical assessment prioritized in decision-making
  • Bacteremia prevalence in this population was extremely low
  • Most bacterial diagnoses were made using imaging techniques, particularly for pneumonia
  • Routine blood tests may have limited utility for this group.

Abstract

The prevalence of bacteremia in well‑appearing children with prolonged fever was extremely low, indicating the limited utility of routine blood tests in this population. Most bacterial diagnoses were based on imaging, especially for pneumonia. Clinical assessment should remain central in decision‑making, reserving investigations for selected cases where they may meaningfully influence management.
