Urethral diverticulum secondary to incomplete surgical repair represents an even rarer complication. We report the case of a 7-year-old boy presenting with post-void dribbling following partial resection of a supernumerary urethra for Effman type IIA2 urethral duplication. Physical examination revealed a patent ventral meatus and a blind dorsal meatus. Voiding cystourethrography confirmed a residual diverticular cavity communicating with the accessory urethra. Complete surgical excision of the residual supernumerary tract was performed via a perineal approach with satisfactory functional outcomes at 3-month follow-up. This case emphasizes that total excision of the supernumerary urethra is mandatory to prevent diverticulum formation. • Urethral diverticulum is a preventable complication of incomplete supernumerary urethra resection • Voiding cystourethrography is the key diagnostic tool in post-surgical urethral anomalies • Complete excision of the accessory urethra is mandatory in Effman type IIA2 duplication • Perineal approach allows safe identification and total resection of residual urethral tracts
