A 6-day-old neonate presented with persistent reticulated violaceous patches from birth, unresponsive to warming, and associated with subtle cutaneous atrophy and limb hypotrophy. The findings were diagnostic of cutis marmorata telangiectatica congenita, distinguished from physiological cutis marmorata by its persistence and associated atrophy. Recognition is important to prompt screening for associated extracutaneous anomalies.
