Splenic artery aneurysm (SAA) is a rare condition, and its spontaneous rupture is life-threatening. It usually occurs in patients with known risk factors such as pregnancy or portal hypertension. We report the case of a 29-year-old male with no prior medical history who presented with sudden abdominal pain and hemorrhagic shock. Contrast-enhanced CT angiography revealed massive hemoperitoneum due to spontaneous rupture of a splenic artery aneurysm. Despite emergency surgical management, the patient died. This case is important because it demonstrates that splenic artery aneurysm rupture can occur in young patients without classic risk factors, highlighting the need to consider this diagnosis in cases of unexplained hemorrhagic shock.
