Abstract: This paper presents a unified pathophysiological interpretation of Polycystic Ovary Syndrome (PCOS) based on the 9-Keel Total Overload Functional Decline (TOFD) theoretical system, with the core axiom of the Cell Innocence Hypothesis and the unified physiological critical constant C (C≈1). It adopts the core formula: (Load × Time) / (Individual Threshold × Cellular Functional Reserve) > C, which is applicable to all chronic diseases, to clarify that PCOS is not caused by immune self-attack, spontaneous cell lesion, or hormone resistance, but a progressive functional collapse caused by long-term continuous overload of ovarian and hypothalamic-pituitary-ovarian (HPO) axis cells exceeding the unified physiological critical constant.
