Abstract

Malignant fibrous histiocytoma also called undifferentiated pleomorphic sarcoma is an aggressive cancer type with a poor prognosis. Primary laryngeal localisation is very rare. It is indicative for radical laryngectomy for prevention of following complications and recurrences. Surgery presents the only one curative chance in case of parastomal recurrence. Creating a new permanent tracheostomy is the aim of the procedure, assisted by some tracheal release or myoplasty procedures. We present a 76 year-old man with parastomal recurrence of malignant fibrous histiocytoma after multiple non-radical laryngeal resections. It was preformed laryngectomy, total thyroidectomy, resection of the proximal third of the trachea and reconstructive stomaplasty. For reconstructive purposes it was carried out a neck defect repair with right pectoralis major myocutaneous flap and a split flap from the medial head of the right sternocleidomastoid muscle for brachiocephalic trunk isolation.
