Abstract

Several studies on lifestyle and psychobehavioral interventions and exercise-based cardiovascular recovery strategies confirm cardiac vulnerability to mental stress, and underscore the importance of integrating behavioral, nutritional, pharmacological, and social perspectives.1-3 This issue of Heart and Mind collects a diverse set of studies that explore novel interventions, causal pathways, and comparative therapeutic strategies across cardiovascular, metabolic, and psychological health. Collectively, these contributions advance our understanding of the complex interplay between mind, body, and social environment across the lifespan. The monocentric, single-arm study by Ke-Lu Yang et al. “Older Patients’ Perspectives on Music Intervention to Reduce Preoperative Anxiety in Cardiac Surgical Procedures: Findings from a Clinical Trial” explored older patients’ perspectives on the acceptability and feasibility of music intervention to reduce preoperative anxiety (PA) before cardiac surgery or transcatheter aortic valve implantation (TAVI). Significant reductions in instrument-measured PA were observed after the first session, with 84.5% feeling more relaxed immediately afterwards. Although reductions were not statistically significant after the second session, 91.1% felt relaxed immediately afterwards. The authors concluded that music intervention is an acceptable and feasible means of reducing PA in older patients undergoing cardiac surgery or TAVI. The study “Causal Effect of 91 Inflammatory Cytokines on Intrinsic Capacity Domains: A Two-Sample Mendelian Randomization Study” by Mengjin Hu et al. investigated the causal relationships between 91 circulating inflammatory cytokines and 5 intrinsic capacity domains (locomotion, vitality, cognition, psychology, and sensory function). The study “Attenuation of Cardiovascular Stress in Beta-Blocker Users Compared to Angiotensin Receptor Blocker Users: Analysis of Recovery Following Treadmill Exercise Testing: A Cross-Sectional Study” by Braga et al. compared cardiovascular recovery after treadmill exercise in 50 hypertensive adults treated with either beta-blocker (BB) or angiotensin receptor blocker (ARB) monotherapy. Patients treated with BB had lower systolic blood pressure at all measured recovery time points (1, 2, 4, and 6 minutes) and a consistently lower rate-pressure product, suggesting reduced myocardial oxygen demand. Absolute heart rates were also lower in the BB group, though heart rate recovery magnitude did not differ between groups. The authors conclude that BB attenuate post-exercise cardiovascular stress more than ARB, likely due to sympathetic blockade. The review “Loneliness and Cardiovascular Disease: A Narrative Review of Epidemiology, Mechanisms, and Clinical Implications” by Hashem and colleagues synthesizes evidence from large longitudinal cohorts showing loneliness is associated with a 15%–30% increased risk of coronary artery disease, heart failure, and stroke. Biological mechanisms include chronic activation of the hypothalamic–pituitary–adrenal axis and sympatho-adrenomedullary stimulationn, leading to endothelial dysfunction, inflammation, oxidative stress, and sleep disturbances. This approach integrates psychological interventions (e.g., music therapy),4 lifestyle and nutritional choices (e.g., type of dietary fat),5 personalized pharmacotherapy (e.g., BB vs. ARB), and recognition of social determinants.6 By dismantling traditional silos seperating cardiology, geriatrics, nutrition, psychology, and primary care, we can move closer to the goal of whole-person health based on an integration of holistic medicine.
