The aim was to determine if cerebrospinal fluid amyloid-beta biomarkers predict future hemorrhagic events in cerebral amyloid angiopathy patients.
Analyzed consecutive patients meeting Boston criteria for probable CAA with cerebrospinal fluid Aβ data.
Primary outcome was incident intracranial hemorrhage; secondary outcomes included ischemic stroke and all-cause mortality.
Used multivariate Cox proportional hazards models to analyze associations with low CSF Aβ biomarkers.
16 (15%) out of 109 patients experienced incident intracranial hemorrhage during 2.93 years follow-up.
Low CSF Aβ biomarkers were independently associated with future hemorrhage (Aβ 40: HR 8.04, 95% CI: 2.43–26.59, p < 0.001; Aβ 42: HR 7.10, 95% CI: 1.58–32.00, p = 0.011).
A composite risk score revealed a high-risk subgroup with a hemorrhage rate of 78% (7/9 patients) and 0% in the no-risk group (0/42 patients).
