In this letter, we evaluated the recurrent morpheaform BCC case presented by Afzal et al., highlighting that perineural invasion and the morpheaform histologic subtype are high-risk features. We emphasized that adjuvant radiotherapy should not be overlooked in high-risk patients with perineural invasion, even in young individuals, despite clear surgical margins. Additionally, we noted that the omission of tumor size reduced the educational value of the case report.
