Pink-White Aesthetic Smile Design with Minimally Invasive Methods: Case Report

Abstract

The aim of this case report is to present a minimallyinvasive pink–white aesthetic smile rehabilitation performedwith diode laser–assisted gingivectomy–gingivoplasty anddirect composite veneer restorations in a young adult patientpresenting with a gummy smile and multiple anterior diastemas. A 27-year-old female patient applied to our clinic with the complaint of an unaesthetic smile due toexcessive gingival display and diastemas between the maxillaryanterior teeth. Clinical examination revealed a gummy smile andmultiple diastemas without periodontal pathology. A minimallyinvasive treatment plan was established, including diode lasergingivectomy–gingivoplasty for teeth 14–24, followed by directcomposite veneer restorations to enhance white aesthetics.Gingival recontouring was performed using a diode laser underlocal anesthesia. After a 2-week healing period, completesoft tissue recovery was observed. After choosing the shade,teeth were isolated using a rubber dam, no preparations wereperformed. Prime&BondUniversal, adhesive (Dentsply Sirona)and a universal resin composite (Ceram.xSphereTECOne,Dentsply Sirona) were applied for restorative procedures andpolymerized using an LED device for 20s. Subsequently, themacro and micro surface morphologies on the buccal surfaceswere created and any occlusal interferences were checked.Finishing and polishing procedures were completed and thepatient was motivated to maintain dental hygiene and wastold about the need for follow-up appointments. 18-monthfollow-up evaluation demonstrated satisfactory estheticoutcomes with healthy gingival contours. Minimally invasive approaches combining diodelaser soft tissue management with adhesive compositerestorations provide predictable, tissue-friendly, and estheticallypleasing outcomes. Considering pink and white estheticstogether is essential for achieving a harmonious and naturalsmile design.
