Abstract The coronavirus disease 2019 (COVID-19) pandemic has negatively impacted both physical and mental health. Accelerated development and free access to the COVID-19 vaccine have led to effective protection against COVID-19 illness. There have been some case reports of post-COVID-19 vaccination psychosis (PCVP) in adolescents receiving different forms of COVID-19 vaccines. Cases have been reported of PCVP after receiving the whole inactivated virus vaccine widely circulated in India. No report has highlighted long-term treatment outcome of PCVP. In this study, we report a case of psychosis with polymorphic presentation in an Indian adolescent with a previous history of a brief psychotic episode, following the administration of first dose of the whole inactivated COVID-19 vaccine. After treatment, all psychiatric symptoms were resolved and laboratory parameters were normalized. In a long-term follow-up, the patient was in sustained remission with regaining of premorbid socio-occupational functioning, emphasizing good prognosis in adolescent onset-PCVP.
