Key Points

  • To evaluate the prognostic implications of nuclear atypia in thyroid nodules classified as Bethesda III.
  • Analyzed thyroid nodules initially classified as atypia of undetermined significance (AUS)
  • Assessed clinical outcomes based on the presence of nuclear atypia
  • Compared outcomes of malignant and non-malignant nodules
  • Clinical outcomes were generally favorable despite the presence of nuclear atypia
  • No significant differences in outcomes based on nuclear atypia
  • Nuclear atypia's relevance appears mainly diagnostic rather than prognostic.

Abstract

Nuclear atypia has been proposed as a useful criterion for subclassifying thyroid nodules with atypia of undetermined significance (AUS), with potential implications for malignancy risk. In our analysis of nodules initially classified as AUS and subsequently confirmed as malignant, clinical outcomes were generally favourable and did not differ according to the presence of nuclear atypia. These findings suggest that while nuclear atypia may have diagnostic relevance, its prognostic impact may be limited.
