Background: Higher incidence of acute coronary syndromes (ACS) in the winter season have been documented. However, the impact of weather conditions on the occurrence of ACS is uncertain. Purpose: The aim of this analysis was to elucidate the correlation between weather conditions (i.e. temperature and humidity) and onset of different types of ACS, Methods: We retrieved ACS hospital admissions to National Health System hospitals in two different large metropolitan areas in Italy (Rome and Milan) between 2010 and 2014. Weather recordings were obtained from public airport meteorological stations in the area of interest. Humidity and temperature (mean, maximum, minimum, daily range, and day-to-day variation -defined as difference between mean temperature of the day compared to the day before) were considered. All data were analyzed as logarithm in order to estimate percent variations using multiple linear regression models. Results: Between January 2010 and December 2014, we identified 93,484 ACS admissions in the area of Milan (16,800 Unstable Angina UA, 29,495 Non ST Elevation Myocardial Infarctions NSTEMI, 47,189 ST Elevation Myocardial Infarction STEMI) in the area of Milan, and 51,236 ACS admissions (11,072 UA, 16
